Medusa first appears during Gaoled Awakening at the Excavation Site as a combat tutorial, then flees after taking damage. Both supplied guide texts separate that scene from Caliginous Depths; defeating her there is not required to find the lair.
If she left during the opening scene, use the southwest Battahl route instead. This page covers the base-game ordinary-world cave, not every later expansion or post-game encounter.
Prepare for the petrifying gaze
