She asks for three miasmite, which the collected guides identify as a drop from phantoms encountered at night. Bring the requested materials back before following the investigation sequence.

Listen to Lubomir and Lottie

Elena introduces you to Lubomir after you return with the materials. Speak to him about his condition, then speak to Lottie when she moves outside. Return to Lubomir after learning about the basement sickroom. This conversation gives you the investigation context; simply entering the building is not the whole objective.

Investigate the Records of Treatment

The records lead to former patients Bruno and Jehan at Walter’s Tavern. Speak to them and follow the information about Elena’s meeting in the common quarter. Go to the marked meeting area at night and approach until the conversation scene plays. This is one evidence lead; it does not replace the medicine investigation.

Ask Radcliff to examine the medicine

Take the Unlabeled Medicine to Radcliff in Checkpoint Rest Town. The archived guides place his home uphill on the southern side of town, behind the apothecary area. Give him the medicine and return after he has had time to examine it. The sources differ on the precise wait, so this guide does not promise a fixed number of hours; his completed analysis is the signal to proceed.

Return and apprehend Elena

Do not arrest her before the investigation

The video explains that the game can allow an earlier arrest with incomplete evidence. An arrest alone is therefore not proof that you have obtained the intended branch. Before taking that action, check that you collected both basement items, followed the former patients’ lead and returned for Radcliff’s findings. These checks are more useful than assuming the quest will stop you from acting too soon.

The IGN guide describes additional care for Lubomir after the quest, but that follow-up is not demonstrated by the checked video.
